Social Worker – Hospital Discharge Team
Location: Plymouth
Hours: Monday - Friday
Working Hours: 8:45am – 5:15pm
Pay Rate: £25.38 per hour (PAYE), paid weekly every Friday
Start Date: ASAP
Contract: Temporary
On-site Only: No remote working
We are recruiting a Social Worker to join the Hospital Discharge Team in Plymouth. The role supports patients admitted to hospital, ensuring safe and timely discharge planning through appropriate pathways.
This is a fast-paced, on-site role requiring full-time presence within the hospital and active ward-based working.
Key Responsibilities
- Complete timely, person-centred assessments and care planning
- Support safe discharge planning, including D2A pathways
- Work within a multidisciplinary team (MDT) and partner agencies
- Apply safeguarding procedures and the Mental Capacity Act (MCA)
- Maintain accurate case recording and documentation
- Promote independence, reablement, and patient-centred outcomes
Requirements
- Qualified Social Worker with HCPC registration
- Strong knowledge of adult social care legislation and safeguarding
- Experience in assessments, care planning, and MDT working
- Understanding of hospital discharge processes (D2A desirable)
- Ability to work at pace in a busy hospital environment
- Strong communication, IT, and record-keeping skills
Additional Information
- Must be based on-site in Plymouth for all working hours
- Set working days each week (no remote or flexible location working)
- DBS check required
